The new UBA Health Plan Survey, powered by Unifi, is now open for UBA Partners to submit plans. To help with data entry, replays of the two training webinars are now available, along with composite equivalent calculators, and access to your 2017 survey data.

You requested, and we listened! You can now access the Health Plan Survey data you entered in 2017 to assist you in entering this year's data. This is not a pull forward – last year's data will not automatically populate into the new survey – but you can review the data you entered last year all in one place in order to make this year's input easier.

How it works:

• On the Wisdom Network, go to the Health Plan Survey page
• Click the blue "2017 Client and FCRG Report" button on the right side of the page
• The clients you entered last year should appear
