So far, Partners have submitted more than 741 surveys, with 62 Partners submitting at least one survey, and 48 submitting two or more. We have a long way to go to reach 20,000, but know that we can make it with your help!

A Microsoft Excel template is now available as an additional method of submitting Health Plan Survey entries. The spreadsheet is an exact replica of the survey questions – same question order, same answer choices you'd encounter in the manual entry method. Download the spreadsheet from the Wisdom Network.

We know that the UBA Health Plan Survey is something our Partners are passionate about. Please consider bringing that passion to the UBA Survey Committee – we're still looking for 6 volunteers to join the committee.
