UBA Health Plan Survey Update

A Microsoft Excel template is now available as an additional method of submitting Health Plan Survey entries. The spreadsheet is an exact replica of the survey questions – same question order, same answer choices you'd encounter in the manual entry method.

You can use the spreadsheet exclusively, or in addition to the online manual entry. If you accidentally enter the same plan using multiple methods, Unifi will discard the duplicate data and count only one plan submission. Download the spreadsheet from the Wisdom Network.

These instructions below are also included in the spreadsheet.

  • Each employee at your Firm who will enter plan data should input all of the plans they are responsible for reporting into a single spreadsheet. For example, if there are 7 employees entering data, the Firm should submit 7 spreadsheets by June 30, one for each of the 7 employees.
  • Each of the responsible employees should submit a single final spreadsheet when done with their workload. The completed spreadsheets can be submitted on different days.
  • Enter Plans in rows:
    • Enter one plan per row
    • Medical Plans are captured in columns AD through DM
    • Prescription Plans are captured in columns DO through FA
    • Retiree Plans are captured in columns FC through FS
    • If a client offers multiple health plans, each plan should be reported on a separate row, one below the other. After completing columns A to AC in the first row, in subsequent rows, skip ahead to column AD.
  • When you have completed a spreadsheet with all the plans you wish to submit, email the spreadsheet to submitHPS@ubabenefits.com
