With only a few weeks left to meet the goal of 20,000 plans, please enter your plans immediately. This is your last chance, so please include your UBA Health Plan Survey data! Remember, Partners are required to enter at least 100 clients or 100 percent of book of business.

The UBA Health Plan Survey can be accessed via the Wisdom Network at Wisdom > UBA Surveys > Health Plan Survey. You can download a user guide with screenshots in order to help you with questions about the survey, or if you are unfamiliar with the process of entering data.
